Product Information:
SEM-P 4mg/3mL is a single-dose pre-filled syringe or ampoule containing Midazolam 4mg in 3mL solution, a short-acting benzodiazepine that enhances the effect of GABA (gamma-aminobutyric acid) in the brain, producing sedation, anxiolysis, anterograde amnesia, muscle relaxation, and anticonvulsant effects.
This higher-concentration formulation is used in hospital, surgical, emergency, or critical care settings for procedural sedation, pre-anesthetic medication, or management of acute seizures—including status epilepticus.
⚠️ Important Safety Note:
Midazolam can cause significant respiratory depression, especially when combined with opioids, alcohol, or other CNS depressants. Use only under continuous monitoring by trained healthcare professionals with resuscitation equipment and flumazenil (benzodiazepine antagonist) immediately available.
Uses of (SEM-P 4MG/3ML):
- Moderate to deep procedural sedation (e.g., endoscopy, colonoscopy, cardioversion, fracture reduction)
- Pre-operative anxiolysis and amnesia
- Acute treatment of prolonged seizures or status epilepticus (IV or off-label buccal/IM)
- Sedation in mechanically ventilated ICU patients (usually diluted and infused)
Key Benefits of (SEM-P 4MG/3ML):
- Rapid onset: IV: 1–2 minutes; IM: 5–15 minutes
- Short duration of action (1–2 hours), allowing quick recovery
- Water-soluble at acidic pH—less painful on injection than diazepam
- Reliable amnestic effect reduces patient recall of distressing procedures
How To Use (SEM-P 4MG/3ML):
- Administered by a qualified healthcare provider via intravenous (IV), intramuscular (IM), or buccal route (off-label for seizures).
- Dosage is individualized and titrated to effect:
- Adults (sedation): Initial IV dose: 2–2.5mg, repeated every 2–5 min as needed (total often 3–5mg)
- Elderly/debilitated: Start with 0.5–1mg IV
- Seizure control (IV): 0.1–0.2 mg/kg (max 10mg)
- Pediatric seizure (buccal/IM): 0.3 mg/kg (common off-label use)
- Inject IV slowly (over ≥2 minutes) to minimize cardiorespiratory depression.
- Continuous monitoring of respiratory rate, oxygen saturation, blood pressure, and consciousness is mandatory.
When Not to Use (SEM-P 4MG/3ML):
Do not use if the patient has:
- Hypersensitivity to midazolam or other benzodiazepines
- Acute narrow-angle glaucoma
- Severe respiratory insufficiency, sleep apnea, or severe COPD
- Shock, coma, or acute alcohol intoxication
- Known history of paradoxical reactions to benzodiazepines
🆘 Avoid in neonates due to risk of "gasping syndrome" from benzyl alcohol (preservative).
Side Effects of (SEM-P 4MG/3ML):
Common: Drowsiness, dizziness, nausea, hiccups, cough, local pain at injection site.
Serious (require immediate intervention):
- Respiratory depression or apnea (most common serious risk)
- Hypotension or cardiovascular collapse
- Paradoxical reactions: Agitation, aggression, involuntary movements (more frequent in children and elderly)
- Prolonged sedation in hepatic impairment or elderly
- In case of overdose or severe depression, administer flumazenil and provide ventilatory support.
Dosage: (SEM-P 4MG/3ML):
- Concentration: ~1.33 mg/mL (same as 2mg/1.5mL)
- Adults: Initial IV dose 2–2.5mg; may repeat
- Maximum recommended single IV dose: ≤5mg in healthy adults; lower in high-risk patients
- Children: Dosed by weight—typically 0.05–0.3 mg/kg depending on indication and route
Who It’s For (SEM-P 4MG/3ML):
- Adults requiring moderate to deep procedural sedation
- Emergency management of acute seizures
- Pre-anesthetic medication in surgical settings
- Critically ill patients under intensive monitoring
Warnings & Precautions:
- Never administer without continuous monitoring and resuscitation capability
- Avoid concomitant use of opioids, alcohol, or other CNS depressants
- Use reduced doses in elderly, liver disease, COPD, or debilitated patients
- Risk of psychological dependence with repeated exposure—limit to acute settings
Drug Interactions:
- Opioids (e.g., fentanyl, morphine): ↑↑ Risk of fatal respiratory depression
- CYP3A4 inhibitors (ketoconazole, erythromycin, ritonavir): ↑ Midazolam levels → prolonged sedation
- CYP3A4 inducers (rifampin, carbamazepine): ↓ Effectiveness
- Always inform your medical team about all medications.
