Product Information:

OPTRA HFA is a pressurized metered-dose inhaler (pMDI) containing Ciclesonide 160 mcg per actuation, delivered via hydrofluoroalkane (HFA) propellant. Ciclesonide is a non-halogenated glucocorticoid prodrug that is activated in the lungs to des-ciclesonide, providing potent anti-inflammatory effects with minimal systemic absorption.

This formulation is used for the maintenance treatment of asthma in adults and adolescents.

⚠️ Note: OPTRA HFA is a preventer (controller) inhaler, not a rescue medication for acute asthma attacks.

Uses of (OPTRA HFA):

1. Maintenance treatment of persistent asthma in patients aged 12 years and older
2. Reduction of airway inflammation, bronchial hyperresponsiveness, and asthma exacerbations
3. Long-term control of asthma symptoms (wheezing, shortness of breath, chest tightness)

How To Use (OPTRA HFA):

1. Shake well before each use.
2. Prime the inhaler before first use or if unused for >10 days (spray 3 test doses into the air).
3. Inhale deeply while pressing the canister; hold breath for 10 seconds.
4. Rinse mouth with water after use to prevent oral candidiasis.
5. Dosage:
6. Adults & adolescents ≥12 years: 1–2 puffs (160–320 mcg) once daily, preferably in the evening
7. Maximum recommended dose: 320 mcg once daily
8. Do not stop abruptly—taper under medical supervision.

When Not to Use (OPTRA HFA):

Do not use if you have:

1. Hypersensitivity to ciclesonide or any excipients
2. Acute asthma attack or status asthmaticus (not a rescue inhaler)
3. Untreated fungal, bacterial, or viral respiratory infections

Side Effects of (OPTRA HFA):

Common: Headache, nasal congestion, throat irritation, cough.

Less common: Oral candidiasis (thrush), hoarseness, dry mouth.

Rare/Serious:

1. Adrenal suppression (with long-term high doses)
2. Reduced bone mineral density
3. Glaucoma or cataracts (with prolonged use)
4. Growth retardation in children (monitor height regularly)
5. Rinse mouth after use to reduce local side effects.

Dosage: (OPTRA HFA):

1. Starting dose: 160 mcg once daily
2. Maintenance: 160–320 mcg once daily based on asthma control
3. Not for children <12 years unless specifically directed

Who It’s For (OPTRA HFA):

Adults and adolescents (≥12 years) with persistent asthma requiring daily controller therapy. Not for intermittent or exercise-induced asthma alone.

Warnings & Precautions:

1. Not for acute bronchospasm—always carry a fast-acting bronchodilator (e.g., salbutamol)
2. Monitor growth in adolescents on long-term therapy
3. Use caution in patients with active tuberculosis or ocular herpes
4. Transfer from oral steroids must be gradual and supervised

Drug Interactions:

1. Strong CYP3A4 inhibitors (e.g., ketoconazole, ritonavir): May increase systemic ciclesonide levels
2. Generally low interaction potential due to localized action
3. Always inform your doctor about all medications.

Additional Information:

Store below 30°C. Do not puncture or expose to heat. Keep out of reach of children. Each inhaler contains 120 actuations. Available by prescription in Pakistan.
