Product Information:

NEXUM IV Infusion contains Esomeprazole Sodium, the intravenous formulation of esomeprazole, a proton pump inhibitor (PPI). It provides rapid and potent suppression of gastric acid secretion by irreversibly inhibiting the H⁺/K⁺ ATPase enzyme (proton pump) in gastric parietal cells. The IV form is used when oral administration is not feasible or rapid acid control is required.

Each vial typically contains 40mg esomeprazole as a lyophilized powder for reconstitution.

⚠️ Note: NEXUM IV is not a ready-to-use solution—it must be reconstituted and diluted before infusion.

Uses of (NEXUM IV INFUSION):

1. Treatment of severe gastroesophageal reflux disease (GERD) with erosive esophagitis when oral therapy is not possible
2. Management of acute gastric or duodenal ulcer bleeding (as adjunct to endoscopic therapy)
3. Prevention of stress ulcers in critically ill, intubated patients
4. Helicobacter pylori eradication (IV phase in severe cases, followed by oral therapy)
5. Control of acid hypersecretion in Zollinger-Ellison syndrome (when oral route unavailable)

3. Administration: Infuse over 10–30 minutes (standard) or as a continuous infusion in high-risk bleeding cases.
4. Standard adult dose: 40mg once daily

When Not to Use (NEXUM IV INFUSION):

Do not use if you have:

1. Hypersensitivity to esomeprazole or any excipients (e.g., sodium edetate, polysorbate 80)
2. Concurrent use of atazanavir or nelfinavir (HIV medications)
3. Severe hepatic impairment (use with caution; max 20mg/day)

Side Effects of (NEXUM IV INFUSION):

Common: Headache, nausea, abdominal pain, diarrhea, injection site reactions.

Rare: Dizziness, rash, flushing during infusion.

Serious (rare):

1. Clostridioides difficile-associated diarrhea
2. Hypomagnesemia (with prolonged use)
3. Acute interstitial nephritis
4. Anaphylaxis (very rare)
5. Discontinue and seek help if severe allergic reaction or persistent diarrhea occurs.

Dosage: (NEXUM IV INFUSION):

1. GERD/Ulcer (non-bleeding): 40mg IV once daily for 3–10 days
2. Stress ulcer prophylaxis: 40mg IV once daily while critically ill
3. Severe hepatic impairment: Max 20mg IV once daily
4. Pediatric use: Not established—avoid unless under specialist care

Who It’s For (NEXUM IV INFUSION):

Hospitalized adults who are unable to take oral medications due to surgery, critical illness, vomiting, or altered mental status, and require immediate and sustained gastric acid suppression.

Warnings & Precautions:

1. Use only when oral PPIs are not feasible
2. Monitor for C. diff diarrhea in prolonged ICU stays
3. Avoid long-term IV use—switch to oral as soon as possible
4. Reconstituted solution is stable for 4 hours at room temperature

Drug Interactions:

1. Atazanavir, nelfinavir: ↓ Absorption—avoid concomitant use
2. Clopidogrel: Minimal interaction (safer than omeprazole)
3. Methotrexate: PPIs may ↑ levels—monitor toxicity
4. Always inform your doctor about all medications.
