Detail Description

Product Information:

MEROGET 1gm I.V Injection contains Meropenem 1 gram, a broad-spectrum carbapenem antibiotic. Meropenem works by inhibiting bacterial cell wall synthesis through binding to penicillin-binding proteins (PBPs), leading to cell lysis and death. It is highly stable against most beta-lactamases, including extended-spectrum beta-lactamases (ESBLs), making it effective against many multidrug-resistant Gram-negative bacteria.

This high-strength intravenous formulation is reserved for severe, life-threatening, or hospital-acquired infections.

Uses of (MEROGET 1GM I.V INJECTION):

  1. Complicated intra-abdominal infections (e.g., peritonitis, abscesses)
  2. Bacterial meningitis (including in pediatric patients)
  3. Hospital-acquired and ventilator-associated pneumonia
  4. Complicated skin and soft tissue infections
  5. Septicemia and sepsis of unknown origin
  6. Febrile neutropenia in cancer patients (as part of empirical therapy)

Key Benefits of (MEROGET 1GM I.V INJECTION):

  1. Broad-spectrum coverage including ESBL-producing Enterobacteriaceae, Pseudomonas aeruginosa, and anaerobes
  2. Excellent penetration into cerebrospinal fluid (CSF)—suitable for meningitis
  3. Lower risk of seizures compared to imipenem (due to minimal affinity for GABA receptors)
  4. Once- or twice-daily dosing possible in some infections

How To Use (MEROGET 1GM I.V INJECTION):

  1. Administered intravenously by a healthcare professional.
  2. Reconstitute with sterile water for injection, then dilute in compatible IV fluid (e.g., 0.9% NaCl or 5% dextrose).
  3. Infuse over 15–30 minutes (standard) or 3-hour extended infusion in critically ill patients for optimized pharmacokinetics.
  4. Standard adult dose: 1g every 8 hours
  5. Meningitis: 2g every 8 hours
  6. Renal impairment: Dose adjustment required (e.g., CrCl <50 mL/min: reduce dose or extend interval)

When Not to Use (MEROGET 1GM I.V INJECTION):

Do not use if you have:

  1. Hypersensitivity to meropenem, other carbapenems, or beta-lactam antibiotics (penicillins/cephalosporins)
  2. History of carbapenem-associated seizures
  3. Severe renal failure without dose adjustment

Side Effects of (MEROGET 1GM I.V INJECTION):

Common: Diarrhea, nausea, headache, rash, phlebitis at infusion site.

Rare: Elevated liver enzymes, thrombocytosis, oral candidiasis.

Serious (rare):

  1. Seizures (especially in renal impairment or CNS disorders)
  2. Clostridioides difficile-associated diarrhea (CDAD)
  3. Hypersensitivity reactions (anaphylaxis, Stevens-Johnson syndrome)
  4. Neutropenia (with prolonged use >14 days)
  5. Discontinue and seek immediate help if severe diarrhea, rash with blistering, or convulsions occur.

Dosage: (MEROGET 1GM I.V INJECTION):

  1. Adults (normal renal function): 1g IV every 8 hours
  2. Meningitis: 2g IV every 8 hours
  3. Children (≥3 months): 10–40 mg/kg/dose every 8 hours (max 2g/dose)
  4. CrCl 26–50 mL/min: 1g every 12 hours
  5. CrCl 10–25 mL/min: 500mg every 12 hours
  6. ESRD/hemodialysis: 500mg every 24 hours (post-dialysis dose may be needed)

Who It’s For (MEROGET 1GM I.V INJECTION):

Hospitalized adults and children with severe systemic infections, especially those suspected or confirmed to be caused by multidrug-resistant organisms. Not for mild or outpatient infections.

Warnings & Precautions:

  1. Reserve for serious infections to prevent antimicrobial resistance
  2. Monitor for CDAD during and after treatment
  3. Use cautiously in patients with seizure disorders or renal impairment
  4. Cross-reactivity with penicillins: ~1–5% risk in allergic individuals

Drug Interactions:

  1. Valproic acid: Meropenem may ↓ valproate levels → risk of breakthrough seizures
  2. Probenecid: May ↑ meropenem levels (not clinically significant)
  3. Always inform your doctor about all medications.

Additional Information:

Store unreconstituted vials below 30°C. After reconstitution and dilution, use immediately or within 4–6 hours at room temperature. Keep out of reach of children. For hospital use only. Available by prescription in Pakistan.
